Three people were injured and two pensioners conveyed to hospital following the collision at 11.40am on Sunday at the junction of Bridge Street and Church Street.
Insp Dyfed Bolton said the crash involved a red Suzuki Alto and a grey Ford Focus.
"The 89 and 91 year-old female occupants of the Suzuki sustained serious injuries and were taken to Morriston Hospital where unfortunately the 91-year-old later died," he added. "Both are from the Llandeilo area and their families have been informed."
